Personalised Nutrition At Your Home - NDIS Participants

Transition Health provides client-centred, personalised, and easy to understand nutritional and dietitian advice in the comfort of your own home. Transition Health is fully accredited by the NDIS to provide accredited practising dietitian consultations.

We cater to the specific and unique needs of NDIS participants, engaging with NDIS support coordinators and other allied health professionals. This ensures we can address a variety of issues which may require changes to one’s diet, nutrition habits, and movement.

Transition Health sits down with participants to discuss their current nutritional intake, make improvements based on their needs and preferences, and encourage further nutritional and health outcomes through movement and exercise known as active sessions. Read more on active sessions here.

Mobile Dietitians - Taking The Stress Out of Healthy Living

We understand that we all lead busy lives - and that includes NDIS participants who may be undertaking a number of activities, including therapy consultations by multiple providers during the week. We provide a mobile dietitian service, catering to South East Queensland: Logan, Brisbane, Moreton Bay, Logan, Ipswich, Gold Coast, Bayside and also via telehealth consultations.

We come to you at a time that’s convenient - be it at your home, clinic, park or outdoor area, or anywhere you or your participant feels most comfortable. We also have availability on the weekends. It’s all part of encouraging lasting health and quality of life through better nutrition!

Mobile Consultations for Food Allergies, FODMAP Diets, Gastrointestinal Issues, Tube Feeding

Transition Health are also dietitians trained in formulating meal plans around intolerances to avoid allergens/foods that exacerbate Irritable Bowel Syndrome as well as nutritious and delicious FODMAP diets. As NDIS accredited dietitians, we assist NDIS participants living independently as well as in supported living environments.

We can also provide expert advice and meal planning for people with gastrointestinal issues such as diverticulitis, Crohn’s, IBD, coeliac disease, and other chronic GI tract diseases.

Transition Health can also formulate nutritional plans for tube feeding, as well as education for carers and family.
