Dealing with diabetes and other chronic health conditions can be difficult for NDIS participants and their carers. There are many “dos” and “don’ts” when it comes to managing diabetes; excluding or including certain foods, managing macronutrient intake, and supplementation of micronutrients where appropriate.
Transition Health's Credentialled Diabetes Educators assist NDIS participants and diabetics with in-home consultations, walking them through what the condition is, creating meal and nutrition plans, and following up with compassionate and thorough accountability and advice.
All our diabetes education and other nutrition plans for remedying metabolic syndrome issues (hypertension, liver disease, heart disease, pre-diabetes, cardiovascular disease) are scientifically-informed, tailored to participants, and simple to understand.
Together, we look at food choices, portion control, decoding food labels, and preparing healthy meals as well as getting more active - and we endeavour to make every session enjoyable and fun!
