We understand that every participant has their own unique needs and goals involving exercise. As NDIS participants may have cognitive, physical, sensory, or intellectual disabilities, Transition Health encourages movement and active sessions suited to the best of one’s ability and aiming for gradual improvement to complement their nutritional journey.
Transition Health creates easy to follow and enjoyable active sessions catering to the wants and needs of every participant. Don’t like running? We find alternatives. We can work on fat loss, muscle strength, bone density, mobility, or fine motor skill retraining - whatever our participants choose, we can help with!
We’re able to come and conduct active sessions in one’s home or assisted living facility, gym or physio clinic, or outdoors. Our client’s comfort is our top priority, always.
