Great question! This service is specifically tailored to clients who are suitable candidates and will be evaluated by a clinician with dual qualifications in personal training and dietetics. Essentially, it’s a review consultation conducted by a dietitian, but with a twist – unlike traditional dietitian consultations where you sit down or do nothing, we incorporate physical activity. This could involve following exercise sessions prescribed by your physiotherapist or exercise physiologist (with their consent and clearance), or it could be a customised session designed specifically for you! It’s important to note that since it’s treated as a dietitian session, you will be billed at the applicable dietitian rates.

An Accredited Practising Dietitian (APD) is someone who has gained nationally recognised university qualifications in Nutrition and Dietetics. This requires a four year university course to build one’s skills as a nutrition scientist.

APDs are registered with their National Board and Dietitians Australia. They are required to complete a minimum of 30 hours of professional development activity (continued learning) annually.

An APD credential is a public guarantee of nutrition and dietetic expertise, in the same way a CPA is a guarantee of an accountant’s proficiency. An APD dietitian is recognised by the Australian Government, Medicare, the Department of Veterans Affairs and most private health funds as the quality standard for nutrition and dietetics services in Australia. It is a recognised trademark and protected by law.

Accredited Practising Dietitians (APDs) help manage various conditions like diabetes, heart disease, and food allergies. We know that everyone’s different, so we look at your medical history, goals, and lifestyle to create personalised plans. We keep up with the latest research and adjust our advice to fit your needs. Making sure our nutrition guidance fits you perfectly is what we do best!

Only Accredited Practising Dietitians (APDs) possess the qualifications to offer medical nutritional advice or conduct clinical nutrition consultations, whether in an individual or group setting. 

Though the term may be used interchangeably, not all practising nutritionists are APDs yet all dietitian have a minimum requirement to be an APD. There is little regulation around a professional using “nutritionist” to refer to oneself – your pet dog could even be called a nutritionist, if they wanted!

At Transition Health, we individualise and personalise every consultation to meet the goals and needs of our clients, using a wealth of nutrition science knowledge and ongoing support to empower our clients to make informed decisions and achieve their best health outcomes.

When you attend your initial consultation, your Dietitian will:
• Discuss the reason for your visit and work together with you to identify and address your health-focused goals
• Take relevant measurements (height, weight, waist circumference) as necessary to form a baseline reading
• Conduct a comprehensive assessment of your nutritional needs and other factors based on the information you provide
• Determine whether it would be helpful to refer you to additional health services
• Help you develop your understanding of food and nutrition to empower you on your health journey
• We will work together to identify simple strategies to achieve both your short and long-term goals, including dietary and lifestyle changes and strategies to overcome any potential barriers
• Provide a detailed and personalised nutrition plan in the form of a report, as well as copies of any resources we have discussed during your consultation (e.g. meal plans, recipe ideas, nutrition information sheets, shopping guides)
• Determine if you require follow-up appointments and their frequency. We may determine if you are appropriate for our active sessions, which means we can do our dietitian consultations while exercising – how cool is that!

During your subsequent consultations, we will:
• Work together with you to review your progress towards and whether your nutrition plan working for you
• Identify where you are succeeding and what difficulties you might be facing in achieving your goals
• Continue to provide ongoing support, strategies and adjustments to empower you to achieve your nutrition and health related goals

  • We also offer ongoing support in between consultations via phone and email so you’re always empowered to make the right nutritional decisions.

Though not necessary, it would be ideal to bring along any medical reports or recent test results as well as a list of current medication. 

If you have diabetes, it would be helpful to bring your Diabetes Record Book containing your blood sugar level records if possible. 

The more information you provide, the more comprehensive our assessment can be, allowing us to offer exceptional and personalised nutrition advice tailored to your needs.

As an approved NDIS provider we can offer assistance to agencies, planned, and self-managed participants under the following support categories:

  • Therapeutic Supports: Improved Health and Well Being
  • Therapeutic Supports: Assistance with Daily Life
  • Therapeutic Supports: Improved Daily Living Skills

These categories can be funded under the capacity building and core support budgets.

Our experienced and accredited NDIS dietitians have worked in various clinical settings, including community facilities, medical centres, and hospitals.

This diverse experience enables us to offer:

  • Nutrition therapy provided for a range of conditions including diabetes, high cholesterol, obesity, malnutrition, gastrointestinal issues, kidney disease, neurodegenerative diseases, mental health challenges, and stroke.
  • Assistance and advice on managing feeding routines for nasogastric (NG) tubes and percutaneous endoscopic gastronomy (PEG), as well as necessary resources and equipment.
  • Support offered for addressing picky or fussy eating habits.
  • Nutrition strategies developed for managing pressure injuries.
  • Guidance provided on fostering healthy eating habits and nutritional education.
  • Assistance available for meal planning, creating recipes, conducting cooking workshops, and organising grocery store tours.
  • Support offered in accessing ready-made meals funded by the NDIS or community programs.
  • Education and guidance provided for caregivers.

At Transition Health, although we see many clients through NDIS referrals, it is not essential to have a referral in order to see a dietitian. We also offer private dietitian and personal training services for people who simply want to improve their fitness and health all the way up to Olympic-level training.

We charge $220 per hour for telehealth or in-home private consultations with an Accredited Practising dietitian.

We offer mobile dietitian and personal training services covering the Sunshine Coast and surrounds, Moreton Bay, Brisbane, Bayside, Logan, Ipswich, the Darling Downs, Gold Coast, and Northern New South Wales. Additionally, we provide telehealth services Australia-wide, as needed.

Yes! Though we mostly cater to NDIS participants, we also offer these services for private clients. Contact us here to discuss pricing and availability. 

We kindly request that any cancellations or rescheduled appointments be made with at least three (3) full business days’ notice. You will receive a reminder call, text, or email a day or two prior to your appointment. If an appointment is cancelled or changed within the timeframe of the original appointment, you will be charged the full consultation fee. 

We recognise that in certain circumstances, short notice cancellations may be unavoidable, and we will exercise discretion in such cases.

Depending on your private health fund, you may be eligible for a rebate under Private Health Insurance. If you are using Private Health Insurance the rebate amount will depend on your particular fund and your level of cover. If unsure, just give your health fund a call to find out if you are covered for dietetics and any applicable gap payments.
