Transition Health uses evidence-based strategies that are personalised to each individual. No two people are the same; we centre our approach around the client, their medical history such as allergies, preferences, and needs in terms of nutrition and support.

Transition Health services South East Queensland, meeting private clients and NDIS participants in Sunshine Coast, Brisbane, Moreton Bay, Logan, Ipswich, Gold Coast, Bayside and also via telehealth.

At Transition Health, our aim is to ensure your experience is not only informative but also enjoyable. We're committed to helping you improve by 1% each week, which adds up to significant progress over time - you'll practically be half a new person each year.

We also offer a unique approach to consultations. Instead of traditional sit-down meetings that can last over an hour, where appropriate, we promote active sessions tailored to your abilities.

We collaborate with NDIS support coordinators and allied health professionals to optimise outcomes for our clients. Our educational resources are tailored for athletes, NDIS participants, and individuals seeking expert nutritional guidance. Furthermore, we provide a wide range of other services.
